Item 21 Attachments
What are they?
Each Form 471 Block 5 Discount Funding Request must include a description of the products or services for which the applicant is seeking discounts. USAC refers to this description as the “Item 21 Attachment”. Item 21 on Block 5 of the Form 471 application is where you provide an attachment number for the description.
Item 21s generally consist of:
- A description or overview of the requested products or services.
- Line item detail that includes the following information: quantity, a description of the product or service, unit cost, and the extended recurring or non-recurring pre-discount cost.
- Additional detail so that USAC can determine if the products or services will be used in a manner that is in compliance with the FCC’s eligibility rules. This information may include: location information for internal connections requests, specific information or specifications for a product or detail of services, copies of a vendors quotes or contracts, and any additional information that will indicate to USAC that the requested items will be used in compliance with the rules of eligibility.
- If there are multiple Billing Account Numbers related to a funding request, those should be included on the Item 21 as well.
Why do you need them?
Item 21 Attachments help with the funding process and provide USAC with information needed to quickly make funding decisions. Incomplete or missing Item 21s will delay the decision process for an applicant’s funding requests. When the funding request is being reviewed, the applicant may be contacted to provide the Item 21 information if it has not been filed in time.
When/how to file
USAC has a new requirement that the Item 21 must be received by the close of the Form 471 filing window. For FY2011 the deadline is March 24th, 11:59 ET. Applicants that fail to submit the Item 21 information by close of the filing window risk a denial because the application was not timely submitted. This new requirement can be found in the directions on the new Form 471 Form.
You can submit item 21s to USAC online, or via email, fax, or US Mail. If you use the Form 471 Wizard within E-rate Manager for Applicants, your Item 21 attachments can be transmitted to the SLD with just a few mouse clicks.
